How do we keep a handle on what’s going on?

As organizations grow in scale, it becomes ever more important to make desired activities and behaviours repeatable to a particular standard.  Witness the host of banking failures and scandals over the last few years for what happens when there's insufficient control.  Control also relates to issues of managing risk and maintaining compliance.

Control doesn't have to be highly formalized - written down as a set of rules governing how work is to be done, by whom and under what conditions - to be effective.  So what are the ways in which we maintain control and compliance without the burden of excessive red tape?  And how do we design our systems so that we avoid traps in how our people perceive business risk?
